Facebook browser access showed a public group post by Karen Ann Macdonald with the visible headline/post text: “MAID death - Kiano Vafaeian was 26 ... I have his death certificate. It says MAID. Had he died in Ontario, it would have said diabetes. Ontario’s College tells doctors the certificate ‘cannot include any reference to MAID or the medications administered.’”

## Media handling

The Facebook post displayed an image that appears to be a B.C. Vital Statistics medical certification/death-registration form. The image contains sensitive personal and medical details. PBC created only a heavily redacted public image for the article and source package. A local-only unredacted copy was used for internal verification and is not published.

Internal visual check found the document appears to be a B.C. Medical Certification of Death and that “MAID/MAiD” is visibly written in the medical cause-of-death area. Personal identifiers and medical specifics are intentionally not transcribed.

## Official source findings

- CPSO Advice to the Profession: Medical Assistance in Dying, last updated April 2026, contains a section “Medical Certificates of Death in the MAID Context.” It says that when completing a medical certificate of death for a person to whom MAID has been administered, the illness, disease or disability leading to the MAID request is to be recorded as cause of death, and “The certificate cannot include any reference to MAID or the medications administered.”
- B.C. MAID page says MAID occurs when an authorized doctor or nurse practitioner provides/administers medication that intentionally brings about death at the person’s request, and lists eligibility/safeguards.
- B.C. death-registration page confirms B.C. death registration creates the legal death record and points health-care providers to the Medical Certification of Death handbook.
- Health Canada annual report page was browser-accessible and confirms MAID data is reported by provincial/territorial health officials and clinicians/pharmacy personnel under national reporting. Direct raw download timed out during capture.

## Limits

- PBC did not independently verify every factual allegation about the individual case, including “doctor shopping,” pharmacy details, funeral-home location, or permission to share the certificate.
- The raw certificate image is not republished because it is a sensitive personal/medical record.
