Japanese booster-dose mortality study: what the 18-million Facebook Reel leaves out
A Facebook Reel says a “horrifying Japanese study” of 18 million vaccinated people proves the more COVID shots you get, the sooner you die. PBC obtained the study. The real paper is serious — but the Reel leaves out the most important limits.

Bottom line
The study is real and worth reading. It found an age-specific pattern in two Japanese municipal datasets: higher-dose groups showed lower all-cause mortality among older adults, but higher all-cause mortality among younger adults receiving five or more doses, and among ages 50–64 receiving six doses.
But the Reel’s slogan — “the more doses you get, the sooner you die” — is not what the paper says. The authors repeatedly caution that the data are exploratory and cannot establish causation.

What the Reel claims
The Facebook Reel from Nutrition with Tom and Lauren says the study tracked “over 18 million people” and that “more doses = higher mortality rate and lower life expectancy.” The thumbnail says: “the more doses you get, the sooner you’re likely to die.”
The Reel then pivots to a “detox guide.” That sales/lead-generation context matters because it compresses a complicated observational study into a fear headline.
What the study actually did
The authors analyzed municipal administrative records from Hamamatsu City and Matsudo City. The paper describes Hamamatsu as having a population of about 780,000 and Matsudo about 500,000. That is not an 18-million-person national cohort.
The records linked residence, vaccination dates and death dates, then calculated all-cause mortality rates by age group and cumulative vaccine dose category. The study looked at all-cause mortality, not vaccine-specific deaths or cause-specific mortality.
What the study found
- Among ages 65–89, higher dose counts were associated with lower all-cause mortality.
- Among ages 20–49, all-cause mortality was higher among people receiving five or more doses than among lower-dose groups.
- Among ages 50–64, mortality was higher in the six-dose group compared with several lower-dose groups.
- The authors describe the findings as age-specific patterns that need further investigation, not proof of direct vaccine-caused death.
Limits the Reel does not tell viewers
- The paper lacked data on comorbidities, healthcare use, socioeconomic status and cause of death.
- The authors say multivariable adjustment and causal inference were not feasible.
- The study is exploratory and hypothesis-generating.
- Biases may include immortal time bias, survivorship bias, healthy vaccinee effect, selection bias and confounding by indication.
- People receiving five or more doses may differ systematically from people receiving fewer doses.
- The study population was limited to two municipalities.
The paper’s own conclusion says definitive conclusions about vaccine safety or effectiveness cannot be drawn.
PBC assessment
This is a legitimate signal paper, especially for the policy question of repeated boosters in younger and non-elderly adults. It supports calls for better linked data, cause-of-death analysis, comorbidity adjustment and transparent risk-benefit evaluation.
It does not support the simple universal claim that every additional dose makes everyone die sooner. The older-adult result runs against that claim, and the authors themselves warn against causal interpretation.
The right takeaway is: real study, real concerning age-specific association, overstated Facebook Reel.
